Membership Application

The following eligibility requirements must be met in order for a practitioner to be eligible for AMPMC, ACNN, ACO membership:

1. Have completed AMPMC, ACNN, ACO application form;

2. Have supplied diplomas obtained from recognized education program;

3. Have supplied transcripts of a minimum of 2,200 hour recognized training program;

4. Have supplied the highest diploma obtained;

5. Have supplied a copy of current Standard First Aid and Level “C” (or higher) CPR Certificates;

6. Have supplied a copy of a Criminal Records Check, inclusive of the Vulnerable Persons Sector completed within 90 days of application;

7. Have supplied a copy of the first page of passport, driver’s license and medicare card;

8. Have supplied a copy of practitioner’s reliability insurance;

9. Have supplied an ID photo.
